Medicaid is a medical insurance available for many people who are in certain income brackets which qualifies them to obtain this government provided insurance. If they do not have Medicaid and also don't have enough cash or another type of health insurance to pay for rehabilitation, they could check if they qualify for Medicaid and then enroll in an alcohol and drug treatment facility that is approved inside their state.
